Call to action
“It is always a good idea to have a conversation with your healthcare professional about your medicines”
Some next steps for you to consider could be:
- Make an appointment to visit your general practitioner (GP) and discuss with them your responses below.
- Share your responses with your local community pharmacist when you pick up your medicines from the pharmacy next.
- Share your responses with your pharmacist who is reviewing your medicines at your home or place of living.
To start a conversation with your health professional about your medicines, you could say:
I don’t like taking . I’m worried it’s giving me side effects, or I don’t think it is working. Could you please review it?
I’m not sure why I take . Could we talk about this?
I would like you to please review if I still need . Could I possibly stop taking this medicine?
For health professionals
Your patient has completed the PRIME tool. This tool is a co-designed, evidence-based conversation-starter tool that empowers individuals and their carers to reflect on their health, medicines, and goals of care to start a conversation with their healthcare professional. It does not provide specific medical advice nor is it a comprehensive medicine review.
Your patient may have shared their PRIME responses with you as they value your input on their medicines. Please feel free to use these reflections during your consultation to review and have a conversation with the patient and/or their carer about their medicines, health and wellbeing.
