The purpose of PRIME tool is to:
- Empower you to start a conversation with your health professional (eg., pharmacist, doctor, nurse) about how your medicines may impact your day-to-day activities
- Make you aware that some medicines may cause side effects.
The benefits and harms of medicines can change over time.
- Your goals of care
- The trade off between the benefits and risks of your medicines.
Reviewing your medicine list can help you consider:
It's time to talk to your healthcare professional about your medicines if:
- You are experiencing side effects from the medicines such as increased confusion, falls, drowsiness or sleepiness
- Your health has changed or worsened
- Your want to take less medicines
- Your medicines are no longer working or making you feel better.
Feel confident about your medicines
You can download the PRIME tool tri-fold brochure to read and fill out in your own time or use the interactive PRIME tool online. You will receive a copy of your responses after completing the tool online. The PRIME tool is available in various languages. After completing the tool, you may choose to share your responses with your health professional.
